Terms of Conduct & Certification
- I am 21 years of age or older or will be by the time
of the Crossroads of America Conference (valid ID must be provided at the
time);
- While I participate in this event I agree to act as a responsible
adult;
- I will not hold the Crossroads of America Conference, Girth & Mirth
Indianapolis, its membership, or any organizations associated with this
event responsible for any loss or injury to my person or property;
- I will not use any nonprescription drugs or illegal substances during
any Crossroads of America Conference activities;
- I further agree by signing the waiver at the time of registration
(when I pick up my CAC ID badge & materials) to release the Crossroads of
America Conference, Girth & Mirth Indianapolis, all its members, associates,
any organizations associated with this event, hotel and staff from any
liability and damages. CAC must have a signed copy of this waiver for every
Conference participant;
- I understand that the Crossroads of America Conference and Girth &
Mirth Indianapolis are not responsible for cancellation or postponement.
(e.g., strike, public emergency, acts of god);
- The Crossroads of America Conference and Girth & Mirth Indianapolis
reserve the right to modify the schedule of events and pricing;
- In attending the Crossroads of America Conference, I understand that
it is my personal responsibility to inform any photographers if I do not
wish my picture to be taken;
- In the event the Crossroads of America Conference is
cancelled due to no fault of Girth & Mirth Indianapolis or it's members,
associates or any organizations associated with this event, Girth & Mirth
Indianapolis is not liable for any non-refundable travel arrangements
I make;
- By registering to attend the Crossroads of America Conference it is
implied that I agree to all these terms and conditions;
Cancellation/Refund Policy

or the mailing address below. Requests must be received by April 1, 2011.
No Refunds after April 1st, 2011. Refunds
subject to a $25 cancellation fee and will be issued by check.
Please allow up to 3 weeks for processing as refunds require
handling by several people to ensure CAC funds are safe & secure.
